ALL release forms MUST be completed, notarized and returned on the date of testing. Failure to return ALL release forms on the date of testing will result in immediate disqualification. You MUST return the following release forms completed and notarized on the date of testing:
1. FOUR copies of the “Applicants Release of Personal Information”
2. One copy of the “ Firearms Qualification test Claims Release”
3. One copy of the “Physical Abilities Test Claims Release”
4. One Copy of the “TCLEOSE Release”
5. One Copy of the “Exam Cover Sheet” form
6. One Copy of the "Range Rules" form
